We appreciate payment for services promptly upon receipt of your bill. Co-pays are expected at the time of service. Medicare: We are participating physicians and accept fee assignment of all Medicare claims. After receiving payment from Medicare, we will file your supplemental insurance claim. Please have policy numbers and insurance company addresses available for our records.

Workers' Compensation: If you have a work-related injury which is covered by Workers' Compensation, we will bill your employer or their insurance company. It will assist us in submitting your claim if you provide us with accurate injury/accident occurrence dates.

Disability Insurance: As a convenience to our patients, we will complete your disability claim form free of charge. Please have the patient's portion of the claim form completed and signed when you bring it to us for completion.

Other Health Insurance: Our office will prepare and submit your insurance claim directly to your insurance company. Remember, however, that you are personally responsible for payment of your bill and payment will be expected in a timely manner. Please present your insurance identification card and any required claim form at the time of your visit. We also ask that you have policy numbers and insurance company addresses available for our records.
